Elkhart Office:

PRN Hospice House Nurse -  RN to work "as needed" in our 7 bed inpatient unit. Flexibility to work various days and shifts required.

Plymouth Office:

South Bend Office:

PRN Hospice House RN - requires flexibility to work various days and shifts.

Medical Director - seeking candidates to work Mon-Fri, 8a-5p, with a shared On Call rotation, in our hospice and palliative care services providing medical care to agency patients. Will also function as liaison to our medical community, interact with IUSOM-SB medical students, local FM residents, students from Notre Dame and Fellows from Mayo Clinic. Ideally, we are seeking IN licensed MDs or DOs who are board-certified or board-eligible in Hospice and Palliative Medicine. Candidates interested in becoming a Certified Hospice Medical Director through successful completion of the HMDCB certification examination, at our expense, will also be considered. While we are seeking a full-time commitment, we will consider part-time positions from interested physicians due to expected growth in patient census. 

Evening Visit Nurse - RN or LPN to work Mon-Fri, 4p-12a, performing scheduled and emergency visits as directed by the triage nurse. 

Hospice/Home Health Aide - CNA to work full-time, day shift.

Hospice House Float Nurse - RN to work Mon-Fri, 4p-12a, in either our Elkhart or South Bend inpatient unit depending on staffing needs. Mileage reimbursement will be provided when traveling between units. Position also includes a $2.00/hr float differential.  

Mishawaka Office:

Admissions Representative - full-time position working 4 days per week from 12:00pm - 10:30pm. Responsibilities include performing pre-assessments and follow-up on referrals. Previous health care experience preferred. Excellent communication skills required.
