Confused, Frustrated or Overwhelmed?
If you are finding it difficult to determine whether or not hospice care would be beneficial for you or your loved one, take a moment to read through some of our most frequently asked questions. If you can't find what you're looking for, please call our Admissions Department at 574-243-3125, and one of our admission representatives will be happy to assist you.

Accessing Hospice Care
Accessing Hospice Care

Patients are usually referred to Hospice by their physicians. However, referrals also can be made by family members, friends, clergy, hospital social workers, discharge planners or others. Patients and families are encouraged to call Center for Hospice Care directly – whether or not they have a physician referral – to seek advice and information at any time during their illness.

If it is believed that hospice care would be beneficial, the Center for Hospice Care admissions team will work closely with all the appropriate people to help facilitate a referral and admission to our program. Meetings to discuss each patient’s unique needs can be arranged at a time and place that is convenient for family.
 

Inpatient Services
Inpatient Services

We have two seven-bed inpatient units - one in South Bend and one in Elkhart. Our Hospice Houses enable patients to receive short-term care in a home-like setting.

Independent Living/Assisted Living/Nursing Homes

Center for Hospice Care provides services wherever our patients call home, including assisted living, residential, group home and skilled nursing facilities. Center for Hospice Care is proud to care for residents in the following facilities:

Hospice services are provided on a scheduled basis by the Hospice Team:

 

  • Patient's own physician and/or the Hospice Medical Director
  • Nurses
  • Social workers
  • Spiritual care counselors
  • Home health aides
  • Trained community volunteers

 

Emergency Care is available 24 hours per day, seven days a week. Hospice is always a phone call away -- (574) 243-3100.

Grief & Bereavement Services
Grief & Bereavement Services

Center for Hospice Care offers a variety of grief and bereavement programs to educate and support people experiencing grief following the loss of someone through death. Our grief and bereavement programs are available at no charge to anyone in the agency's service area.

Book Club

Center for Hospice Care recommends the following books, which provide information and insight on a wide variety of topics related to death, dying, grief and loss. Often, those who are grieving receive insufficient assistance from friends, family and society. It is important to have additional resources during this traumatic time.

Palliative Care
Palliative Care

If a patient has made the choice to pursue aggressive or life-extending treatment for a progressive or incurable illness, and desires pain management or symptom control at home, the Center for Palliative Care may be a suitable choice.
