Our Service Promises to You
  1. As the area’s first and most experienced hospice program, we will provide your patients and families with the most comprehensive range of end-of-life care solutions.
  1. To provide the best care and expertise for your patients, our three full-time, board certified Hospice and Palliative Medicine physicians will work with you to the extent you desire.
  1. A CHC staff member will directly answer your call 24/7 whenever you call 1-800-HOSPICE*. There is no answering service.
  1. Your patients will have access to the area’s only free-standing inpatient hospice facilities for symptom management, crisis care, and/or respite care. 
  1. Dedicated CHC interdisciplinary staff are standing by 24/7 to provide emergency nursing, psychosocial, spiritual and emotional support to your patients and families.

Hospice Care
Hospice Care

Patients are usually referred to Hospice by their physicians. However, referrals also can be made by family members, friends, clergy, hospital social workers, discharge planners or others. Patients and families are encouraged to call Center for Hospice Care directly – whether or not they have a physician referral – to seek advice and information at any time during their illness.

If it is believed that hospice care would be beneficial, the Center for Hospice Care admissions team will work closely with all the appropriate people to help facilitate a referral and admission to our program. Meetings to discuss each patient’s unique needs can be arranged at a time and place that is convenient for family.
 

Palliative Care Consultation
Palliative Care Consultation

The Center for Palliative Care’s consultation program is specifically developed to address the complex symptom management needs of patients with advanced serious illnesses. The program is staffed by Nurse Practitioners in collaboration with our physician medical directors. Our program is structured to enhance the care you provide by offering expert evaluation of your patient’s needs along with suggestions to assist you in managing those needs. Following a consultation, we report back to you with our observations and recommendations. You continue managing your patient’s care.

Palliative care is a holistic approach to caring for patients with potentially life-limiting illnesses. We can help improve the quality of life for patients and their families by addressing the discomforts and stressors commonly associated with serious illness.  Your patients with an advanced serious illness affecting their quality of life or causing them significant discomfort may benefit from a palliative care consultation.

Specialty Programs for Hearts, Lungs & Dementia

Center for Hospice Care has developed innovative programs to help meet the needs of our patients, including HeartWize for advanced heart disease, BreatheEasy for COPD, and DementiaCare. Each program addresses the unique needs of patients affected by these diseases.
Diseases of the heart and lungs now comprise 38% of the diagnoses for the more than 2,000 patients seen by CHC each year. Our specialty programs take an interdisciplinary approach and include the use of emotional and spiritual support for the patient and family.

Grief Counseling

Center for Hospice Care offers a variety of bereavement programs to educate and support people experiencing grief following the loss of someone through death. Bereavement programs are available at no charge to anyone in the agency's service area.
